Checklist for Types of Scaffold, Etc. – (Sample) For scaffold – (Note 1)

This table shows an example of the form of the checklist. Prepare a checklist for each type of scaffolding such as prefabricated scaffolding, single-pipe scaffolding, wedge binding scaffolding, overhanging scaffolding, suspended scaffolding, shelf scaffolding, and rolling scaffolding. Also prepare a checklist for work platforms and erection corridors according to their structure and use.

The construction period should be the actual construction period, not the contracted construction period. Note that the results of inspections, etc. shall be stored until the work using scaffolding or work platforms is completed, according to Article 567, Paragraph 3, Article 575-8, Paragraph 3, Article 655, Paragraph 2, and Article 655-2, Paragraph 2 of the Ordinance on Industrial Safety and Health.

The person who conducts inspections shall be appointed from among those who have sufficient knowledge and experience in inspecting scaffolding, such as those who have attended the training for improving the competence of chief operators of scaffolding based on Article 19-2 of the Industrial Safety and Health Act.

Provide the reasons for the inspection, including details of whether the inspection is to be conducted due to strong winds, heavy rain, heavy snow or other inclement weather, earthquakes of medium intensity or greater, after scaffolding assembly, after partial dismantling, or after alteration, as stipulated in Article 567, Paragraph 2, of the Ordinance on Industrial Safety and Health. Also, if periodic inspections are conducted, provide the details.

In the “Purpose, Type, and Outline of Scaffolding, etc.” area, describe the name of the scaffolding, such as prefabricated scaffolding for exterior work, rolling scaffolding for interior work, suspended scaffolding for ship painting, etc., so that its use and structure are clear. Also indicate the size of the scaffold (height x width, number of layers x number of spans) and the outline of the installation surface, etc.

Inspection items should include, at a minimum, the items specified in items 1 through 9 of Article 567, Paragraph 2 of the Ordinance on Industrial Safety and Health. In addition to the statutory items, the following items may also be added to the inspection items: confirmation that the scaffolding is in accordance with the scaffolding plan, elevating equipment, maximum load indication, and so on.

Specific details of the inspections, as shown in the attachment “(Sample) Details of Inspections,” must be provided to ensure that inspections related to the above inspection items are conducted accurately. The details of the inspections shall be determined through discussions not only with the business operator but also with the principal contractor, temporary construction equipment manufacturers, etc.

The inspection results, good or bad, shall be described in such a way that the relevant part of the scaffolding is clearly indicated.

Details of corrections made must be clearly described, including the location of correction, method of correction, and date of correction.

Corrections must be checked not only by the inspector, but also by the manager, operator, or a person acting on his/her behalf.

When inspecting fall prevention equipment such as handrails, middle rails, etc., it is necessary to carefully inspect not only whether or not they have been removed or fallen off, but also whether or not they are properly installed.
